SUBMISSION AND PARENTAL CONSENT FORMS

The 2009 LOL Kids Comedy Night Submission Form and Parental Consent Form must be completed and returned to or faxed to (403) 270-9438 or mail LOL Kids Comedy Night, c/o Foothills Academy, 745 37th Street N.W., Calgary, Alberta, T2N 4T1.

The Submission Form and Parental Consent Forms are located under the Submissions Tab on the website.

Submission Form and Parental Consent Forms are to be received on the same day as the video submission. We recognized that in some instances, these documents may need to faxed or mailed. No video submissions will be accepted for the competition until Foothills Academy receives a complete Submission Form and Parental Consent Form.

Any Submission or Parental Consent Forms received after Monday, October 19 at 4:00 pm MST will not be permitted to participate in the 2009 LOL Kids Comedy Night competition.

PARENTAL OR GUARDIAN CONSENT

The 2009 Laugh Out Loud Kids Comedy Night (the "Event") is an all Minors Event. If a participant is under 18 years of age, he/she is considered a minor in the province of Alberta. All Minor’s must submit a Parental Consent Form, signed by the parent or legal guardian, before acceptance to participate in the Event.
For good and valuable consideration, I the "parent/guardian" hereby grant to Stampede Entertainment Inc ("Producer") and Foothills Academy Society ("Client") its agents, affiliates, licensees, successors and assigns the following non-exclusive rights, but not the obligation:

I, as the parent/legal guardian of the ("Minor"), hereby consent to Minor’s participation in the Event. I understand that the Event involves a personal video recorded submission, the possibility of a live performance, and video content for production at the Event.
In connection with Minor’s participation at the Event, I give the Producer, the right to film, photograph and record Minor’s name, nickname, pseudonym, picture, biographical material, silhouette, voice and/or likeness (collectively "Appearance"). This material can be used for media in connection with the Event, and the advertising and promotions including all forms of television, web, radio and other advertisements, promotions and publicity to broadcast, reproduce, distribute and exhibit such photographs and/or recordings, by all means of media, devices, process and technology.
I, on behalf of Minor, hereby acknowledge that if selected as a finalist, the Minor’s appearance at the Event may constitute a public performance. That this is a charity fundraising event and that Minor shall not be entitled to any compensation for such appearance, not to any guild or union payments for use or reuse of such appearance.
Producer has consent to incorporate any photographs, audio-only or audiovisual recordings in which Minor appears (as outlined in paragraph 3) as well as historical documentation and/or archival purposes, including without limitation, Foothills Academy Society website. I, on behalf on Minor, waive any right to any additional compensation on the reuse of these recordings.
You acknowledge that Producer is the exclusive copyright owner of the Program and your Appearance. You release Producer and agree not to institute any claim against Producer, such as any claim for invasion of privacy, infringement of your right of personality, defamation, infringement of your moral rights and/or any other personal or property rights, in connection with Producer’s use of your Appearance.

You represent and warrant that you are fully authorized to grant the rights provided for herein and the use by Producer of your Appearance will not infringe the rights of any person. You understand that the Producer is proceeding with the production of the Event in reliance upon and induced by this Personal Release.

VIDEO SUBMISSION DEADLINE

Deadline for video submission is Monday, October 19 at 4:00 pm MST. Any submission past this due date will not be permitted to participate in the LOL Kids Comedy Night competition.

No submissions will be accepted for the competition until Foothills Academy receives a complete Submission Form and Parental Consent Form.

CONTEST JUDGING

All submissions will be judged by a committee selected and approved by Foothills Academy.

The panel of judges will consist of Foothills Academy personnel, the event producer, local celebrities and event sponsors.

A formal criteria judging sheet will be used for each submission. Judging results will not be available to the public and destroyed after the results are finalized.

Only three winners will be selected to open for Bill Cosby at the LOL Kids Comedy Night on Monday, November 2 2009. Show time at 7:00 pm.

PUBLIC VERSUS PRIVATE VIDEO FILES

All YouTube video files are to be posted on YouTube for Private Viewing. This is to protect the contestant and Foothills Academy.

Any video submissions that are accessible as a Public File may be disqualified from the competition.

Foothills Academy WILL NOT be responsible for any submissions posted for public viewing before or after the November 2, 2009 competition.

CONTESTANT WINNER TERMS

Three winners will be contacted Monday, October 26 2009 by phone and email. These winners will then be posted on the Foothills Academy website. Our apologies, but as a not for profit organization, we only have the infrastructure to contact all the winners.

Winners must be available to appear at the Southern Alberta Jubilee Auditorium the afternoon of Monday, November 2^nd for sound check and rehearsal.

One parent or guardian will be permitted to accompany the contest winner back stage at the rehearsal and performance. Due to limited space, we can only permit one parent or guardian.

A brief biography of the winners will be required for the event program. Winners must submit this biography to cvancamp@foothillsacademy.orgCLICK by Wednesday, October 28 by 4:00 pm MST or they will be disqualified from the competition.

The contest winner will be escorted, and supported on stage, by the event host Dave Kelly from CityTV’s Breakfast Television show.

The Event Producer may request at anytime, an interview with contestant for recording purposes and audio visual content for the night of the event or any other public engagements.

Winners will be required to wear a lapel microphone as a back-up to a stand-up microphone for their performance. It is recommended that winners take the time to practice speaking into a microphone before their performance.

STANDARDS OF VIDEO SUBMISSION

YouTube Posting Submission

All entrants are encouraged to submit their video through YouTube. Due to privacy issues, Foothills Academy WILL NOT accept any other on-line video platform applications for submissions. This is to protect you the user.

Mail Submission

Where absolutely necessary, entrants can submit their video by mail in a DVD format. All mail submissions become the sole property of Foothills Academy. Due to limited resources, DVD’s will not be returned to entrants.

Content Guidelines

Submissions containing any NUDITY, PROFANITY OR DISCRIMINATION or any other content determined unsuitable by Foothills Academy will be immediately disqualified from the competition. This is a family event and there will be NO acceptations.
Submissions will be judged on content – the comedian and the performance value. However, entrants are encouraged to produce a video that is of first-rate quality – efficient lighting, good level of sound and clear video quality.

STANDARDS OF CONTENT

Formats for your video

YouTube will accept any video in QuickTime or any other wmv file.

Otherwise, the following settings will give the best results for good a good quality video. We ask that you please stay within these parameters.

Video Format: H.264, MPEG-2 or MPEG-4 preferred
Aspect Ratio: Native aspect ratio without letterboxing (examples: 4:3, 16:9)
Resolution: 640x360 (16:9) or 480x360 (4:3) recommended
Audio Format: MP3 or AAC preferred
Frames per second: 30
Maximum length: 10 minutes (we recommend 2-3 minutes)
Maximum file size: 1 GB

HOW TO UPLOAD YOUR VIDEO AND SUBMIT YOUR ENTRY

STEP 1: First you will need to log into YouTube at .

STEP 2: Create a new user account or use an existing YouTube user account.

Navigate to Sign Up tab top of the page
Create User Name - recommend first initial and last name
Fill in location, Postal Code and Date of Birth
Check the box that states "Let others find my channel on YouTube if they have my email address"
Accept Terms of Use
Enter current email address with password

STEP 3: Navigate to the "Upload Video" tab on this page.

STEP 4: File Name: Type in the "file name" of video submission in the pop up box from your computer.

STEP 5: Title Box: Type in LOL plus your last name. For example, "LOLsmith" would be the title if your last name           was Smith.

STEP 6: Description Box: Type in a theme or title of your video submission.

STEP 7: Tags Box: Type in "LOL Kids Comedy Night Foothills Academy Submission".

STEP 8: Category: Select "Comedy".

STEP 9: Privacy: Click the "Private" button. Video only accessible to you and up to 25 people at your discretion.               Due to privacy reasons this video submission IS NOT TO BE A PUBLIC FILE.

STEP 10: Save Changes: Select "Save Changes"

STEP 11: Video File Upload: Please confirm that your video selection has been saved. The next page will                 confirm "Success" or "Failure". If a "Failure" happens please review the Download Video Format             section of the Upload Video Document.

STEP 12: Send Video to Foothills Academy

Go to "Account" tab on your YouTube Account
Click on "My Videos" and then "Upload Video"
Click "Play " video to confirm video can be played

STEP 13: Copy the URL address posted in the right hand box on the page
Paste the URL into an email
Email the video link to: .

STEP 14: Include your full name and phone number in the title of the email.

STEP 15: Include Submission and Parental Consent Form as an attachment

If you have scanning options, please include a signed copy of the Submission and Parental Consent Form in the video submission email. If not, please indicate when and how (fax or mail) these forms will be sent.
